List of Documents Needed For Student Enrollment

1. Immunization Records

2. Proof of residency ( Electric, Phone, Gas or similar bill to name of the parent or guardian)

3. Copy of the Applicant's Birth Certificate

4. Copy of the Applicant's Social Security Card

5. Record's Release Form ( Can also be provided in the office )

6. Previous School Records ( Should be obtained from the most recent school )

7. Official Test Scores ( Ohio Proficiecny, IOWA, Stanford, PSAT, SAT, ACT, etc. )

8. Emergency Medical Authorization Form ( Can also be provided in the office )

9. RTA Forms ( Can also be provided in the office )

10. Signed Copy of Student and Parent Committment Form
