Student Work Permits at HSA Cleveland High School
Under Ohio law, all students under the age of 18 are required to obtain a work permit to be legally employed during the school year.
At HSA Cleveland High School, we’re here to help you through the process of securing your work permit so you can start your job with everything in place. Follow the steps below to get started:
Steps to Obtain a Work Permit
Step 1: Pick Up or Print the Application
Students may either:

Print the work permit application from the Ohio Department of Commerce website, or

Pick up a blank application from the school’s main office.

Step 2: Complete All Sections of the Application
The application consists of three parts that must be completely filled out:

Parent or Guardian Section

Employer Section
-
Physician Section
Please note:

No sections or boxes may be left blank.

You must include the maximum number of days you expect to work, the earliest start time, and the latest end time on the employer portion.
Step 3: Return the Completed Forms to the School
Once all three sections are complete:

Bring the entire application back to the school.

School staff will upload the necessary information to the Ohio Department of Commerce to generate your official work permit.
Important: The student must be present to sign the work permit at the time it is issued.

Work permit applications can be dropped off at the beginning of the school day, and students may return at the end of the day to pick up the completed forms.
During the summer, students may visit the school during regular office hours.
Please note: you may need to wait while your form is processed, so plan accordingly and allow enough time before the office closes.
